Wellness Sessions by the Community Health Teams

Your Way to Wellness

Your Way to Wellness is a six week program, for people living with a chronic condition, such as arthritis, chronic pain, or diabetes. Led by people who have chronic conditions, participants will learn current information about healthy living, share ways to manage, and take charge of your life – all in a relaxed, supportive, social environment online.
