The Nova Scotia Green Sleeve is a green folder that acts as a central place to keep copies of important documents which are used to communicate a patient’s health care plans, goals, and preferences to healthcare providers. This can include what is important to the patient culturally and spiritually. Green Sleeves are NOT only for patients being followed by specialist palliative care. They are intended for anyone with a serious life-limiting illness across care settings and the age continuum and are provided to patients by healthcare providers.

Examples of forms that can be kept in the Green Sleeve include:

  • Personal Directive
  • Nova Scotia Health Patient-Centered Priorities and Goals of Care form
  • Emergency Health Services Special Patient Program care plan
  • Nova Scotia Health Expected Death at Home form

Patients should be advised to keep their Green Sleeve on or near the fridge at home and to bring their Green Sleeve to all medical appointments, including visits to the emergency department and hospital admissions. When forms are updated, a copy should be placed in the Green Sleeve and the patient reminded to take their green sleeve home.
