The Serious Illness Care Program has developed a system to assure appropriate communication with seriously ill patients based on proven palliative care best practices.

The Serious Illness Care Community of Practice is a private space for collaboration between interdisciplinary teams implementing the Serious Illness Care Program at their institutions. This virtual community provides implementation support through peer mentoring, sharing of experiences and lessons learned, downloadable materials, videos, and webinars.
