Provider Profiles

Part of the Three-Factor Framework process involves understanding the skills, competencies and scope of employment required to support or meet patient, client, community care need(s). Once provider requirements are understood, a team-based care model comprised of appropriate health care roles can be determined and resourced. To support this, leaders can locate applicable professional practice terms, provider profile resources and/or associated health care professional links that help provide an overview of the various roles that may form part of a team-based care model and the parameters in which various roles are able to perform or practice.

 

Provider Profile Resources

The minimum data to be reviewed includes:

  • Current Complement Review:
    • Classifications and numbers of all team members (direct care providers only)
    • Leadership roles (Charge RN; Team Lead; resource nurse etc.)
    • Years of practice experience breakdown (i.e., 0-1 yrs.; 1-2 yrs; 2-5 yrs; 5-10 yrs; 10-15 yrs; 15-20 yrs; 20-25 yrs; 25-30 yrs; > 30 yrs)
    • Years of experience in current practice environment
    • Post-entry-level education and/or certifications
    • Levels of care provider experience (novice/evolving/expert)
    • Accommodations
  • Scheduling Practices
    • Scheduling considerations
    • Staffing mix (for example, RN/LPN roles, Care Team Assistant or other support roles)
    • Planned staffing/actual staffing/budgeted staffing (See Planned Staffing Template below)
    • Percentage FTE/PTE (full-time equivalent)/part-time equivalent)
    • Staffing variance analysis (for example, leaves, vacancies, OT, sick time, purchased services)
    • Contingency staffing (for example, casuals/floats/agency)
    • Other interprofessional team members (role and %FTE, consult services or part of core complement)
