Team-Based Care Design

Tools and resources leaders can use to help support the review, design or redesign of team-based care models.

Integrated Review

Review findings must be integrated to ensure all components have been analyzed to understand how they are interrelated and/or dependent on each other. Review components should not be viewed in isolation. This supports and informs a comprehensive analysis of:
  • Structures (for example, provider mix and complement, care delivery model)
  • Process/practices (for example, safety huddles, patient rounding, patient assignment guidelines, provider transition to practice plans, etc.) needed to ensure appropriate team-based care delivery to optimize patient/client care and outcomes.

Integrated Review Examples

Below are examples of what an integrated review could look like once the data has been collected and reviewed. Key considerations for implementing a redesigned or newly designed team-based model have also been included.
