Review findings must be integrated to ensure all components have been analyzed to understand how they are interrelated and/or dependent on each other. Review components should not be viewed in isolation. This supports and informs a comprehensive analysis of:
Structures (for example, provider mix and complement, care delivery model)
Process/practices (for example, safety huddles, patient rounding, patient assignment guidelines, provider transition to practice plans, etc.) needed to ensure appropriate team-based care delivery to optimize patient/client care and outcomes.
