Team-Based Care Design

This guide contains resources and tools for health care leaders developed and curated by Interprofessional Practice and Learning (IPPL). These resources can be referenced or applied as leaders review, redesign or develop a team-based care model and/or care processes (for example, purposeful rounding or care planning) that support role optimization, team functions and person-centered care.
