A complete Team-based Care Review should occur when:
- There are changes to services.
- There are changes to the patient population (for example, a planned or sustained trend of more than 3 months in changes to patient care needs, intensity, complexity or acuity of care).
- A new unit/care setting is being planned.
A modified Team-based Care Review should occur every quarter, using the principles of the Three-factor Framework. The review should include:
- A quick practice environment scan (engage Team Lead/Interprofessional team) to determine if there have been any recent trends in different patient care needs that require:
- different skills/competencies
- review of associated impact on the workload/intensity of care requirements.
- A quick scan of patient Case Mix Groupings (CMGs), ADT (Admission/Discharge/Transfer) data to assess for variance from what was expected.
- A review of risk management data trends over the previous 3 months and analysis of “root cause” for findings. This is done by engaging providers in the review process and required practice environment process updates/revisions.
- A review of provider profile information, including financial utilization data (for example, purchased services; overtime trends, current leaves/vacancies, and anticipated timeline for mitigating variance between planned and actual staffing).
- A review of trends or needs to redeploy staff in or out of the practice setting.
- A review of onboarding and transition to practice progress for new providers and learning and development opportunities for existing providers.
