Team-based Care

Team-based care is the provision of health services to individuals, families, and/or communities by two or more health care providers. Team-based care entails health care providers working to understand, meet and/or support the care needs of patients/clients using their education, certification, skills, experience, and scope of employment. The team works collaboratively with patients and their caregivers (to the extent preferred by each patient) to accomplish shared goals and achieve coordinated, high-quality care.

Team-based care and the appropriate design and implementation of a team-based care model is to be based on the Three-factor Framework which includes a review and consideration of:

  • optimize provider and team practice(s)
  • optimize patient satisfaction
  • optimize staff satisfaction and overall engagement of staff
