Team-Based Care Design

Tools and resources leaders can use to help support the review, design or redesign of team-based care models.

Implementation

Once it has been determined that a redesign or revisions to a current care model is required, it is important to develop an implementation plan. If you require support to develop a plan, engage internal unit/portfolio resources (Physician Leaders, Quality Leaders, IPPL, etc.) in the process.

Remember: An implementation plan is not static. You will need to re-evaluate the plan at various intervals or stages and amend accordingly.

Components of an Implementation Plan

Key components of an Implementation Plan include:

  • Identify who needs to be involved and what will be their role.
  • Develop goals and objectives: this will help to determine the outcomes that must be evaluated.
  • Identify key evaluation activities and measures: What type(s) of evaluation is required (process/formative/summative)? What data sources are required? How do we get the data? Who can help with evaluating data?
  • Assess PE and provider readiness.
  • Assess and identify short-, medium-, and long-term activities/actions and associated timelines.
  • Consider and incorporate change management practices (for example, what will be communicated to the team and how, how do team members engage in the plan and in delivery of key activities, etc.).
  • Adopt a Plan-Do-Study-Act process to evaluate changes and make revisions as you go.
