Key Messages

  • We need to improve access and the flow of patient care within our facilities. We have created a Care Coordination Centre, known as C3, that uses a command centre model to better coordinate decisions to improve patient experience and care delivery.
  • C3 is generating actionable insights that will enable us to continuously improve how health care is delivered, to improve the quality of patient care.
  • C3 supports teams to identify issues and find solutions using real-time data. This will translate into:
    • Improved patient access and flow
    • Efficient bed management and discharge planning
    • Better scheduling of staff
    • More collaboration between health zones across the province

Overview

C3 is the first province-wide, command centre model in Canadian health care. It will directly support the six solutions identified in Action for Health, the Government’s strategic plan for the health care system.

C3 is improving how we coordinate the movement of patients throughout their health care journey. It allows us to give Nova Scotians the best care possible by:

  • Providing a real-time view of access to care, patient flow and health system resources to support decision-making
  • Creating more efficient processes and collaborative teams
  • Implementing advanced technology  

C3 will result in better access to care for patients and more efficient use of resources.

Current Activities

We have established C3 at the QEII Health Sciences Centre. We are implementing Tiles (similar to apps) in Central Zone that provide us with the technology to access real-time data to support decision making.

C3 is implementing six GE HealthCare Tiles:

The following Tiles are currently live:

  • Capacity Expeditor
  • Patient Manager
  • Boarders Expeditor

Tiles will be become available to other areas of the province in 2023.
